Ongoing Care Partner Support
For many families and care partners, one conversation brings clarity, but dementia continues to change over time. Ongoing support provides a space to return, reflect, and navigate new situations as they arise.
Ongoing Care Partner Support provides continued, steady guidance, helping you reflect on what is happening, consider new possibilities, and strengthen communication and connection.
Support may include:
Talking through real-life situations that feel difficult or confusing
Exploring what may be happening beneath the surface of interactions
Finding clarity when decisions feel uncertain or overwhelming
This support is typically offered virtually for consistency and accessibility, allowing me to work with families across the U.S. and Canada.
Guided Engagement Sessions | In Person, Packaged-Based
Guided Engagement Sessions are in-person visits focused on meaningful, relationship-centered interaction with the person living with dementia.
These sessions are shaped in the moment based on the person’s needs, abilities, interests, and responses with the goal of supporting comfort, engagement, and meaningful connection.
Care partners may be a part of these sessions when appropriate. Following the session, I provide reflection and guidance to help care partners understand what was noticed and how it may inform future interactions.
Sessions may take place in the home or care community.
Ongoing Support Pathways
Over time, some families find that a combination of consultation and in-person engagement best supports their needs.
When helpful, care is structured to include both reflective guidance and direct engagement sessions, based on evolving needs.
Plans are determined together after the initial consultation.
